Do Neuropathy Supplements Work? What ALA, ALC and B12 Show
Do neuropathy supplements work? Only in narrow cases. Vitamin B12 helps when you are low. Alpha-lipoic acid has the best results by vein, and acetyl-L-carnitine has very low certainty. Guidelines favor prescription medicines and exercise, so supplements are an add-on to discuss.

Do neuropathy supplements work, ingredient by ingredient?
Do neuropathy supplements work? Each popular one has a narrow story. None has been shown to help most people with nerve damage.
Read the table by asking two questions. Who was studied, and how was the dose given?
| Supplement | Best evidence | Catch |
|---|---|---|
| Alpha-lipoic acid (ALA) | By vein, 300-600 mg for 2-4 weeks eased diabetic symptoms (Linus Pauling Institute, Lipoic Acid) | Oral trials weaker; many studies from one funded group |
| Acetyl-L-carnitine (ALC) | About 9 points less pain on a 100-point scale (Cochrane, Acetyl-L-carnitine 2019) | Very low certainty; avoid during chemotherapy (ASCO Post, CIPN guideline 2020) |
| Vitamin B12 | Fixes numbness when you are deficient (NIH ODS, Vitamin B12) | Little reason to expect help if levels are normal |
Why do guidelines favor medicines over supplements?
Guidelines favor medicines because medicines have stronger trials. Supplements are not among the recommended treatments.
The American Diabetes Association lists pregabalin or duloxetine as first choices for neuropathic pain (ADA, Diabetic Neuropathy Position 2017). The American Academy of Neurology lists four drug classes, advises against opioids, and allows exercise, tai chi or mindfulness (AAN, Painful Diabetic Neuropathy 2022).
The cause comes first. In type 1 diabetes, intensive glucose control cut clinical neuropathy by 64% over about five years (DCCT, Annals of Internal Medicine 1995). A B12 shortage needs its own fix (NINDS, Peripheral Neuropathy). No supplement has results like these.
Do not stop or change a nerve, pain or diabetes medicine because of a supplement. Ask your care team before starting one.
Does Nervecept contain ALA, ALC or B12?
No. Nervecept contains none of the three. Its label lists glucosamine sulfate, boswellia, chondroitin, turmeric, quercetin, methionine, MSM and bromelain.
These are mostly joint-style ingredients sold with nerve claims. The biggest line is glucosamine at 500 mg, against 1,500 mg a day in the knee trial (Clegg et al., NEJM 2006 (GAIT)). No trial has tested Nervecept for neuropathy.

The label also says it contains shellfish (crayfish). If you want to look closer, the ingredients page covers each line. Bottles start at $49 each on the 6-bottle pack.
When should neuropathy symptoms get care first?
Get care first when symptoms are sudden, one-sided or spreading. A supplement should never delay a diagnosis.
Ongoing numbness, burning or weakness also deserves a visit, since the cause decides the treatment.
- Sudden one-sided numbnessArm, leg or face, on one side of the body
- Face drooping, slurred speechOr trouble understanding speech
- Bladder or bowel changesLost control with back pain or numb inner thighs
- Weakness that spreadsGetting much worse over hours or days
- A foot sore that will not healOr a red, hot, swollen foot
SoonTingling, burning or numbness in the feet or hands that lasts more than a few weeks, keeps you up at night, or comes with diabetes, heavy drinking or chemotherapy: book a doctor visit and ask about B12 and blood sugar tests.
Call 911For sudden numbness or weakness on one side, a drooping face, trouble speaking, or sudden loss of bladder or bowel control.
Causes and warning signs from NINDS (NINDS, Peripheral Neuropathy) and MedlinePlus (MedlinePlus, Sciatica); foot sores from CDC (CDC, Your Feet and Diabetes).
FAQ01
Nervecept questions people ask
Is alpha-lipoic acid or acetyl-L-carnitine better for neuropathy?
Neither is proven better. ALA has stronger results by vein, while ALC has very low certainty (Linus Pauling Institute, Lipoic Acid) (Cochrane, Acetyl-L-carnitine 2019). Ask a clinician whether either suits you.
Does vitamin B12 help neuropathy if my levels are normal?
Evidence is lacking. B12 helps nerve symptoms mainly when you are deficient (NIH ODS, Vitamin B12). A blood test shows your level.
Can neuropathy supplements replace gabapentin or duloxetine?
No. Guidelines list those medicines as proven options, and supplements are not (AAN, Painful Diabetic Neuropathy 2022). Never stop a prescription without your prescriber.
Can acetyl-L-carnitine be harmful?
It can be. For people with chemotherapy nerve problems, an oncology panel says to avoid it because data suggest harm (ASCO Post, CIPN guideline 2020). Ask your cancer team first.
